At Heart of Humanity, we are proud to provide home care and nursing services through California’s Self-Determination Program (SDP) — offering individuals greater choice, control, and flexibility in how their services are planned and delivered.

What Is the Self-Determination Program?

The Self-Determination Program is a voluntary alternative way to receive Regional Center services that gives eligible participants and their families more freedom and control over support services. Unlike traditional models where the Regional Center selects service providers for you, SDP participants are empowered to:

• Develop a person-centered plan that reflects their personal goals and priorities.

• Receive an individualized budget based on assessed needs.

• Choose the services and providers that best meet those needs.

• Manage how public funds are spent within their budget and related to their Individual Program Plan (IPP) objectives.

The program is designed to support participants’ independence and quality of life by giving them control over who provides their care, how services are delivered, and how funding is used.

How It Works

1. Orientation & Planning: Participants attend required SDP orientation sessions and work with a person-centered planning team to define goals and develop an IPP.

2. Individual Budget: Participants receive an individualized budget and create a spending plan that outlines the services and supports they choose.

3. Provider Selection: With that budget, participants select qualified service providers that align with their plan. You can view the full list of providers online here.

4. Ongoing Management: Participants maintain responsibility for managing their services and budget with support from a Financial Management Services (FMS) provider.
